Should it be necessary for the student, parent(s), and/or guardian listed above to receive medical care while participating with Global Arts and/or any activities with which it is affiliated, I/we hereby give Global Arts personnel permission to use their judgement in obtaining that care. I/We also give permission to the physician selected by Global Arts personnel to render medical care that s/he deems necessary and appropriate. I/We understand that Global Arts has no insurance covering medical or hospital costs incurred by any participant and, therefore, any cost incurred for such treatment will be entirely my/our responsibility.

I consent to Global Arts and its evaluation team obtaining access to my school and juvenile court records, if applicable. I agree to allow my child's school to release its school records including grades, attendance, and discipline records to Global Arts. I understand that the GALA student will complete a confidential survey about his or her attitudes and opinions as part of an evaluation. I understand the information obtained will be statistically analyzed and that my child's name will be withheld for confidentiality purposes.

I fully understand that transportation to and from Global Arts’s field trips and other sanctioned activities is provided by only properly licensed and insured commercial carriers, with Global Arts named as an additional insured. I further understand that Global Arts staff members or teachers are never permitted to transport my child in any other vehicle, except in the event of an emergency. I authorize Global Arts Executive Director, or their designee, to exercise their judgment in determining the existence of a transportation emergency and its safest and most logical resolution.

I consent to my child’s participation in Global Arts’ music education program. I agree to hold Global Arts harmless against any liability, loss, or expense I incur or suffer as a result of participation in the music education program, including but not limited to, loss, injury, illness, disease, or expense in traveling to and from classes and program events. I understand that, as with any activity, there are risks. I therefore agree to assume these risks and to hold Global Arts harmless from any action or actions, suit or suits, in law or equity, which may be brought by any person or persons in connection with, or with reference to, the administration, planning, preparation, development, conduct, and execution of Global Arts’ programs. I understand that my child will participate in Global Arts’ music education program from the date of this agreement until withdrawn.

I give permission and consent for the child named below to be photographed, interviewed, or video recorded during the entire Global Arts program. I further give permission and consent that any such media may be published via any media medium (i.e. print, broadcast, internet, or other available medium). Their name may be named in the media, and may be attached with photographs/interviews/video recordings.

Global Arts takes the responsibility for the care and safety of your children very seriously. During their scheduled class time, parents should rest assured that Global Arts teachers and staff will take full responsibility for the supervision of their students. However, we do expect parents or guardians to be available to drop-off and pick-up their children in a timely manner.

Therefore, it must be clearly understood that Global Arts is only responsible for the supervision of your child from when they check-in with a staff member to when they check out with a family member or when they return to another program. If, for whatever reason, a student does not check-in with Global Arts, they are not under our supervision. Global Arts is not responsible for the supervision of your child before or after the child’s scheduled class time in our community-based music program.

Please remember that each parent/guardian is to ensure that his/her child arrives and departs safely from their Global Arts classes. As a community, we value the safety of our students and work with our families to provide a safe and nurturing environment for them.
